How recently your passport was issued changes everything
There isn't one universal passport name change form. If your current passport was issued within the last year, a name change is generally handled on Form DS-5504, and there's typically no government fee. If it was issued longer ago but you still qualify to renew by mail, the change goes on your DS-82 renewal. If you don't qualify to renew by mail at all, it goes on Form DS-11, in person.
Getting this wrong is expensive in time, not just money. A misfiled application means your certified document comes back weeks later, unprocessed. We settle it before anything is prepared.

Photocopies and notarized copies are generally not accepted. The document must be a certified copy issued by the appropriate authority. We confirm what you have before you mail anything.
